SECOND-DEGREE ASCENDANT (GRANDPARENT) WHO POSSESSES, OR POSSESSED AT THE TIME OF DEATH, EXCLUSIVELY<\/span> ITALIAN CITIZENSHIP<\/strong><\/p>\n

To submit an application, applicants must provide the following:<\/p>\n

    \n
  1. APPLICATION FORM<\/strong> (MOD B<\/strong> see Forms<\/strong><\/a>) duly filled in but not signed<\/strong>. The form must be signed in person<\/strong> before a consular officer on the day of the appointment. The relevant fee for the legalization of the signature must also be paid (art. 24 \u2013 see Fees<\/b><\/a>);<\/li>\n
  2. ORIGINAL CERTIFICATES<\/strong> of birth<\/span>, marriage\/civil union<\/span>, and death<\/span>\u00a0(if applicable) of the Italian grandparent<\/strong> of the applicant issued by the relevant Italian Municipality – or Municipalities. Note:<\/strong> Make sure the birth certificate is a full version that includes the parents’ names;<\/em><\/li>\n
  3. HISTORICAL RESIDENCY CERTIFICATE<\/strong> issued by the relevant Italian Municipality (or Municipalities) listing all residency addresses<\/u> of the Italian\u00a0grandparent <\/strong>from birth to present<\/u>;<\/li>\n
  4. DECLARATION OF NON-NATURALIZATION<\/strong>* issued by the authorities of every country where the grandparent<\/strong> resided (as indicated in the Historical Residency Certificate). For the United Kingdom, the declaration must be obtained from The National Archives for the period 1844\u2013September 1986, from the Home Office UK Border Agency for the period after October 1, 1986. The declaration MUST explicitly include all variations and spellings of the names as they appear in the rest of the documentation.\n
